Care Management can carry the weight of day-to-day issues for families needing on-going assistance or monitoring of care. We begin with an on-site needs assessment conducted by our Care Team, which may include a Social Worker, Registered Nurse or Physician. After consultation with the client and family, we will locate available resources, make recommendations for care options and coordinate services. Once a plan has been implemented, we can visit the client frequently to assure continued high standards of care.

We work with attorneys, bank trust officers and Certified Professional Guardians who are responsible for vulnerable adults. Our primary goal is to advocate for the client so that the highest possible quality of life is maintained.
